(1514) Wraparound Facilitator

About the role

The Wraparound Facilitator provides high-fidelity, family-driven, youth-guided Wraparound facilitation and care coordination services under the supervision of the Clinical Program Manager. The Wraparound Facilitator partners with youth, families, natural supports, providers, schools, and community agencies to identify strengths and needs, facilitate Child and Family Team meetings, develop and monitor individualized Wraparound Plans of care, coordinate services, and support families in achieving their goals.

Responsibilities

The Wraparound Facilitator promotes collaboration, community integration, crisis prevention and planning, and the development of sustainable natural supports while ensuring services are delivered in accordance with YES Waiver, agency, contractual and state requirements. This position is responsible for coordinating services across multiple systems, monitoring progress toward identified outcomes, advocating for youth and families, and ensuring continuity of care throughout enrollment in the YES Waiver program. The Wraparound Facilitator maintains timely and accurate documentation in the electronic health record, manages multiple priorities and deadlines, and complies with all productivity, quality, and documentation standards.

Requirements

• Minimum of 55% of working hours must be dedicated to direct service delivery
• Work schedules may include evening hours, as needed, to accommodate the needs and availability of youth and families
• A valid Texas driver’s license is required, along with the ability to maintain approved driver status throughout employment

Qualifications

• Bachelor’s degree in Human Services
• Strong understanding of mental health diagnoses and treatment
• Ability to work both independently and collaboratively in a fast-paced, recovery-oriented environment
• Proven ability to build meaningful connections with individuals served

Preferred Qualifications

• Master’s degree in Human Services or a related field
• At least one year of experience providing wellness and recovery support services in mental health, substance use treatment, and/or crisis intervention and screening
• Clinical licensure in counseling, behavior analysis, social work, family therapy, or a related discipline
• Bilingual proficiency in Spanish
• Experience with concurrent (real-time) documentation practices
• Current licensure as a Licensed Chemical Dependency Counselor (LCDC)

Pay

Hourly Salary Range: $22.22 – $28.47. Differential pay available for bilingual staff ($0.75/hour), associate or advanced licensure (up to $1.00/hour), tenure with BTCS (up to $4.00/hour), and LCDC licensure ($0.50/hour). Annual salary range: $46,217.00 - $59,217.00/year
