About the role

Capital Health is a dynamic health care resource accredited by the DNV, offering comprehensive patient care across two hospitals, an outpatient center, satellite ED, and a network of primary and specialty care. This role is within Capital Health Medical Group, a provider network of over 600 physicians and other providers.

Responsibilities

  • Delivers quality care through the nursing process of assessment, diagnosis, planning, implementation, and evaluation.
  • Affirms patient safety by executing appropriate policies and procedures.
  • Supports pain management and pharmacological measures, assisting patients in identifying individualized comfort measures.
  • Mobilizes resources in complex cases to maximize patient control and participation.
  • Provides information and interpretation of the patient's condition and offers coping mechanisms.
  • Formulates and documents a discharge plan that maximizes patient's ability to continue meaningful life activities.
  • Anticipates patients' needs and ensures handoff communication.
  • Identifies and addresses issues related to patient education for medication management.
  • Provides discharge planning which includes instructions on discharge medications.
  • Uses discretionary judgment to modify patient care regimens.
  • Facilitates appropriate responses from other health care team members.
  • Attends educational sessions to improve departmental needs.
  • Participates in unit-based and hospital-wide committees.
  • Engages in performance improvement activities.
  • Participates in efforts to achieve successful Patient Satisfaction scores.

Requirements

  • Education: Associates degree or graduation from an accredited school of nursing.
  • Experience: One-year related experience or training.
  • Certifications: AHA BLS - Healthcare Provider, Registered Nurse - NJ, ATCN or TNCC, NIH Stroke Scale Certification, NRP, PALS.
  • Knowledge and Skills: Strong problem-solving and decision-making skills, interpersonal skills, excellent verbal and written communication, quick adjustment to change, knowledge of nursing principles, and a good work ethic.

Qualifications

  • Knowledge of nursing skills, hospital practices, procedures, and standards.
  • Usual Work Day: 12 hours.
  • Sensory Requirements: Very accurate 20/40 vision, near and far vision, color discrimination, accurate depth perception, and accurate hearing.
  • Physical Demands: Frequent physical demands include Push/Pull, Twisting, Bending, reaching forward, reaching overhead, Squat/kneel/crawl, Wrist position deviation, Pinching/fine motor activities, Keyboard use/repetitive motion, Taste or Smell, Sitting, Climbing, Standing, Walking, carrying objects, talking, and hearing.
