Provide one-on-one guidance, support, education, referrals, coordination of care, and other assistance to patients as they move through the healthcare continuum.

Responsibilities

• Develops relationships with participating physicians and service providers.
• Liaising with medical providers.
• Guiding patients to appointments.
• Coordinating the logistics for plan adherence-appointments.
• Supporting patient family members.
• Other duties may be assigned.

Requirements

• High School Diploma or GED equivalent.
• Ability to be scheduled for work based on operational needs of the hospital.
• Possesses the ability to deal tactfully and harmoniously with guests.
• Demonstrate effective communication skills with all stakeholders (e.g., patients, employer staff, payer contacts).
• Ability to work in compassionate, supportive and collaborative ways with patients and families in an acute medical setting.
• Good interpersonal and communication skills with the ability to handle the pressures related to serious life crises and chronic disease situations.
• Cognizant of environmental factors, infection control issues and maintains a safe environment.
