PACE Program
The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Responsibilities
- Works as part of an interdisciplinary team to help older adults maintain and improve their mobility, strength, balance, and overall physical function so they can remain safe and independent in their homes and communities.
- Assesses each participant’s physical abilities, develops individualized treatment plans, provides therapeutic exercises and mobility training, monitors progress, adjusts care based on needs, and educates participants and caregivers on safe movement, fall prevention, and appropriate home strategies.
- Collaborates with doctors, nurses, occupational therapists, and other team members to coordinate care and support quality of life for frail elderly participants enrolled in the PACE program.
Qualifications
- Licensed Physical Therapist (PT) in the state of Kansas
- Graduation from an accredited physical therapy program
- Experience working with older adults or geriatric populations
- Knowledge of rehabilitation principles, mobility training, and fall prevention
- Ability to perform physical assessments and develop individualized treatment plans
- Strong communication and documentation skills
- Ability to work effectively as part of an interdisciplinary healthcare team
- CPR certification (or ability to obtain)
Preferred Experience
- In a PACE program, home health, long-term care, or skilled nursing setting
- Familiarity with chronic disease management and age-related conditions
- Experience providing caregiver education and patient training
- Knowledge of adaptive equipment and assistive devices
- Strong organizational and time-management skills

Pay
$1,500.00 sign-on bonus to be paid out in two increments, at 6 months and 1 year of employment.
