Youth Visitation and Mentor Coach - Sullivan
About the role
The Family Advocacy & Mentorship (FAM) Program supports youth (ages 0–21) and their caregivers through an integrated, strength-based model. The program is led by a dedicated team of case managers, youth coaches, and a parent partner. FAM provides tailored services such as rapid response, case management, mentorship, caregiver education, life skills development, and access to essential resources. Our approach prioritizes family engagement, safety, stability, and overall well-being through direct support and community-based advocacy.
The Youth Coach is a key member of the FAM Team who provides direct support, family visitation support, and mentorship to at-risk youth.
