About the role

The Youth Psychiatric Transition Specialist will focus on establishing relationships with local psychiatric facilities to enhance the timeliness and duration of engagement in Care Coordination. They will act as the primary point of contact between inpatient psychiatric hospitals and the Care Management Entity (CME) to ensure seamless transitions for youth moving from higher levels of psychiatric care to community-based services.

Responsibilities

  • Maintain a consistent physical and/or virtual presence within inpatient psychiatric units to ensure real-time awareness of admissions, treatment planning, and anticipated discharges.
  • Navigate hospital systems and collaborate with multidisciplinary clinical teams to support youth transitioning from psychiatric care to community-based services.
  • Build and maintain strong relationships with hospital staff, fostering trust and effective communication in high-pressure environments.
  • Proactively identify and address system barriers, presenting practical solutions to improve care coordination and patient outcomes.
  • Work independently while maintaining alignment with CME supervision and team structures, utilizing electronic health records, tracking systems, and agency-issued technology.

Requirements

  • Licensed by the State of Ohio as a Counselor, Social Worker, Marriage and Family Therapist (LCSW, LCSW-C, LMFT, LPC, LPCC).
  • Relevant experience in behavioral health, care coordination, hospital discharge planning, or a related human services field.
  • Experience working with youth with serious emotional disturbance, psychiatric conditions, or complex behavioral health needs.
  • Strong relationship-building, communication, and organizational skills.
  • High emotional intelligence and the ability to work effectively in high-pressure environments.
  • Proactive problem-solving skills, including the ability to identify and resolve system barriers.
  • Ability to work independently while maintaining alignment with CME supervision and team structures.
  • Knowledge of Ohio behavioral health systems, Medicaid services, and community-based resources.
  • Valid driver’s license, reliable transportation, and ability to travel within the service area.

Qualifications

  • Bachelor’s degree plus 2 years of relevant experience, or Master’s degree plus 1 year of relevant experience.
  • Relevant experience in behavioral health, care coordination, hospital discharge planning, or a related human services field.
  • Experience working within hospital, psychiatric, behavioral health, or care management systems preferred.
  • Knowledge of Ohio behavioral health systems, Medicaid services, and community-based resources preferred.
