Youth Personal Supports Counselor
About the role
The Youth Personal Supports Counselor provides individualized, developmentally appropriate support to youth and transitional-aged youth enrolled in Rock Creek Foundation's Youth Personal Support Program. Working across family homes, foster homes, independent living settings, schools, and community environments, this role delivers direct one-on-one support guided by each individual's Person-Centered Plan. All services are youth-centered, trauma-informed, culturally responsive, and family-inclusive.
Essential Duties And Responsibilities
- Provide individualized one-on-one support to youth ages 13 and older with developmental disabilities, emotional and behavioral challenges, or co-occurring needs
- Deliver HIPAA-compliant virtual support to maintain or improve functional abilities, strengthen social connections, enhance meaningful relationships, and promote participation in age-appropriate activities and community life
- Provide in-person overnight support where required
- Support youth in building skills across all core development areas including social and communication skills, activities of daily living and personal care, safety awareness, emotional regulation, budgeting, healthy routines, and self-advocacy
- Absently participate in Person-Centered Planning processes and contribute meaningfully to individualized goal setting
- Work alongside youth, caregivers, schools, providers, and community partners to reinforce skill development across environments
- Maintain accurate and timely documentation of all services in accordance with DDA requirements, Community Pathways Waiver guidelines, and RCF policies
- Ensure all services align with the youth's approved Person-Centered Plan and all applicable program limitations
- Comply with all applicable regulations, organizational policies, and ethical standards at all times
Experience And Qualifications
- High School Diploma (or equivalent) and/or one (1) year of related experience
- Knowledge of Internet and Microsoft Office software required
- Must be able to be trained on Electronic Medical Record software
- Must be able to successfully pass the Maryland Board of Nursing CMT course, as well as other DDA core trainings
- Must possess a valid driver's license and proof of current automobile insurance
- Frequent local travel is required as part of the day-to-day responsibilities of this role
