Position Qualifications (Minimum qualifications are required unless stated otherwise.)
- Experience in psychiatric, community mental health, or health care setting in direct care provision including individual, family, and group treatment.
- Experienced in using measures for establishing baselines and monitoring data registry outcomes for common conditions like depression, anxiety disorders, PTSD, substance use disorders.
- Proven knowledge of community, outpatient behavioral health, and SUD treatment resources.
- Proficiency in care coordination, managed care contact and discharge planning.
- Knowledge of community and state agency as well BH / SUD treatment resources.
Education and Training
- Master’s degree in clinical social work or related healthcare field required.
Licenses, Certifications & Registrations
- LCLILM - Licensed Independent Clinical Social Worker in MA or LHMC Licensed Mental Health Counselor in MA.
Other Requirements
- CPR Certified.
- Effective communication skills with patients, family, physicians, ancillary providers.
- Ability to form rapid engagement and alliance building with patients.
- Ability to maintain and monitor confidentiality of patient records.
- Ability to work in an independent and self-directed manner.
- Ability to learn new duties and adjust to change.
