Responsibilities

• Complete a head‑to‑toe body audit on all new admissions and re‑admissions to identify open areas; ensure skin sheets, treatment orders, and preventative measures (T&P, specialty mattress, heels up or boots) are in place.
• Review residents with new skin alterations and ensure implementation of skin sheet, treatment, specialty mattress, Braden scale update, T&P orders, heels up orders or boots, and initiate care plan.
