Wellness LPN or LVN Nurse

Responsibilities

• Assist in maintaining a physical, social and psychological environment in the best interest of residents.
• Maintain and administer treatment plans prescribed by a physician.
• Monitor and assess residents' health and wellbeing.
• Cook and prepare meals for residents as needed.
• Coordinate with providers and pharmacies to ensure continuity of care.
• Notify the resident care coordinator or executive director of any issues related to treatment plans.

Requirements

• LPN or LVN License Required per state regulations.

Qualifications

• Bachelor's degree in Nursing preferred.
• Experience working with elderly individuals preferred.

Skills

• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of medical terminology and procedures.
• Basic computer skills.

Base pay in range will be determined by applicant’s skills and experience.
