About the role

Professional Case Management, a leader in home healthcare, is seeking Registered Nurses in Tennessee to provide in-home healthcare services. The role offers one-on-one patient relationships, enhanced client health, quality of life, and peace of mind. Schedules are customizable to fit personal commitments such as attending school, traveling, volunteering, or spending time with family and friends.

Responsibilities

  • Initiate actions independently to improve client care
  • Assign and delegate functions based on education and demonstrated competence
  • Collaborate with clients, families, and others to establish realistic goals of care
  • Evaluate health status and implement interventions to support client needs
  • Administer prescribed medications and IV therapy, or provide education on self-administration
  • Develop and document a plan of care, including client education and counseling
  • Evaluate home environment for safety and stability
  • Document interventions and client responses accurately and according to Agency guidelines
  • Participate in team meetings and clarify reporting responsibilities
  • Perform other duties as assigned

Qualifications

  • Graduate of a state-approved school of professional nursing
  • B.S.N. preferred
  • Current, unrestricted license as a Registered Nurse in the state(s) of practice
  • New graduate nurses are welcome to apply
  • Current CPR certification
  • Current TB test

Essential Functions

  • Initiate actions independently and responsibly to improve the quality of client care
  • Perform client visits to provide skilled interventions
  • Act as client advocate
  • Assess and evaluate health status of assigned clients
  • Collaborate with client, family/others, and other members of the healthcare team
  • Initiate appropriate preventative and rehabilitative nursing procedures
  • Practice nursing with respect for individual, cultural, and spiritual differences
  • Communicate interventions and client responses effectively
  • Report significant changes in client condition and needs to physician and other members of the team
  • Maintain client records and clinical notes systematically
  • Coordinate services for the client as needed
  • Participate in team meetings and clarify individualized reporting responsibilities
  • Document interventions and client responses accurately
  • Initiate delegation of tasks to only those with verified skills and only for stable clients
  • Participate in ongoing, professional self-development
  • Perform other duties as assigned
