About the role

As an On-Call Nurse, you will have the autonomy to utilize your nursing skills, providing care to patients and families after hours during urgent times. You will be invited into patients' homes and lives during uncertain moments, becoming a trusted face and making a meaningful difference.

Schedule

  • Saturday and Sunday: 4:30 PM – 5:00 AM
  • One 10-hour shift during the week: 4:30 PM – 3:00 AM
  • Work 34 hours, paid for 40

Responsibilities

  • Assess physical, psychosocial, and spiritual needs of the patient and family (RN only).
  • Gather data related to patient and family’s physical, psychosocial, and spiritual needs (LPN).
  • Provide intermittent skilled nursing services under the patient’s physician’s direction.
  • Monitor and evaluate pain and symptoms; assist patients and families in managing those problems.
  • Serve as a liaison to the patient’s physician(s) when needed.
  • Provide emotional support to the patient and family.
  • Refer patients and/or family members to other services when appropriate.
  • Provide health care teaching to the patient and family as related to care in a residence.
  • Make visits to the patient’s residence at the time of death.
  • Maintain, promptly complete, and return all required documentation.
  • Communicate and act as a liaison with patients, family, and health professionals while respecting confidentiality and dignity.
  • Relay pertinent information to appropriate team members.
  • Participate in meetings as initiated by the team leader.
  • Participate in mandatory or elective RC University events, trainings, or in-services.
  • RNs shall participate in triage coverage as needed.
  • Perform other duties as assigned.

Requirements

  • High School Diploma or equivalent required.
  • Graduate of an accredited school of nursing; Bachelor’s degree preferred.
  • Licensed RN or LPN in the state of Ohio.
  • Experience preferred in pain and symptom management, oncology, or mental health.
  • Ability to communicate effectively, orally and in writing.
  • Ability to work independently and collaboratively with a team.
  • Demonstrated knowledge and skills to provide appropriate care to all age groups.
  • Ability to handle stressful and constantly changing conditions; must make sound professional decisions quickly.
  • Ability to access patients and families in any residential and/or public setting in any weather condition (e.g., climbing stairs or snow-filled walkways).
  • Proficiency in Microsoft Office suite, particularly Word and Excel, and use of shared team files, donor databases, grant portals, and virtual meeting technology.
  • Valid driver’s license, good driving record, and auto insurance at a level acceptable to Reserve Care and/or its insurer.

Physical Requirements

  • Ability to stand and walk for extensive periods of time.
  • Manual and finger dexterity and eye-hand coordination for nursing care and operating equipment.
  • Heavy work requiring exertion of up to 75 lbs. of force occasionally and 50 lbs. frequently, utilizing safe transfer techniques and body mechanics.
  • Ability to stoop, squat, or bend frequently.
  • Corrected vision and hearing within normal range to observe and communicate with patients and staff.
  • Possible exposure to environmental elements such as disease, bloodborne pathogens, chemicals, pet dander, allergens, inclement weather, and difficult patient/family situations.

Conditions of Employment

  • Compliance with Annual Flu Shot Policy or ability to provide exemption documentation.
  • Active auto insurance policy as a licensed driver in Ohio with proper coverage.
  • Active driver’s license.
  • Proof of eligibility to work in the United States.
  • High level of integrity, ethics, and professionalism.
