About the Role
Join Moses Lake Assured Home Health, a part of LHC Group and the Optum family of businesses, as a Registered Nurse in Home Health. You will provide and direct nursing care to patients in their homes as prescribed by the physician, ensuring compliance with applicable laws, regulations, and agency policies. This role involves coordinating the total plan of care with other health care professionals to maintain continuity of patient care.
Responsibilities
- Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other health care professionals.
- Supervise care provided by home health aides and licensed practical/vocational nurses, provide instruction, and assign tasks according to State and federal regulations.
- Provide required supervisory visits and document patient visits per policy and payer requirements.
- Complete comprehensive assessments (OASIS), including medication reconciliation, accurately and timely.
- Ensure patients meet home health eligibility and medical necessity guidelines, determine primary focus of care, and develop the plan of care within State guidelines with the physician.
- Communicate relevant information timely and effectively with appropriate agency staff, physicians, patients, and family members.
- Take direction from the Clinical Director and Executive Director, complete assigned tasks, and assist in orienting new personnel.
- Actively participate in survey readiness activities, performance improvement plans, and initiatives to reduce unnecessary patient hospitalizations and improve patient safety.
- Participate in on-call and weekend rotation as needed to meet patient needs.
- Adhere to and participate in the agency's utilization management model.
Requirements
- Current and unrestricted RN licensure in the state of Washington.
- 1+ years of Home Health experience.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR Certification.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

- Ability to work independently.
- Solid communication, writing, and organizational skills.
Pay
The hourly pay for this role will range from $36.27 - $54.41 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, and certifications.
