About the Role
As a Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes as prescribed by the physician, ensuring compliance with applicable laws, regulations, and agency policies. You will coordinate the total plan of care with other healthcare professionals to maintain continuity of patient care.
Responsibilities
- Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
- Supervise care provided by home health aides and licensed practical/vocational nurses, including instruction and task assignment per State and federal regulations.
- Provide required supervisory visits and document patient visits per policy, payer requirements, and LHC policy.
- Deliver high-quality clinical services within the scope of practice, infection control standards, and in coordination with the healthcare team.
- Complete comprehensive assessments (OASIS), including accurate and timely medication reconciliation.
- Ensure patients meet home health eligibility and medical necessity guidelines, determine the primary focus of care, and develop the plan of care with the physician.
- Communicate relevant information timely and effectively with agency staff, physicians, patients, and family members regarding patient care, visit assignments, schedule changes, and coordination with other clinicians.
- Take direction from the Clinical Director and Executive Director, complete assigned tasks, and assist in orienting new personnel.
- Actively participate in survey readiness, performance improvement plans, and initiatives to reduce unnecessary hospitalizations and improve patient safety.
- Participate in on-call and weekend rotations as needed to meet patient needs.
- Adhere to and participate in the agency’s utilization management model.
Requirements
- Current and unrestricted RN licensure in the state of practice.
- 1+ years of Home Health experience.
- Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR Certification.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Pay
This role offers a $10,000 sign-on bonus. The hourly pay ranges from $28.27 to $50.48 per hour based on full-time employment, depending on factors such as local labor markets, education, work experience, and certifications. We comply with all minimum wage laws as applicable.
