Weekend Registered Nurse

About the Role

The RiverStone Health Hospice Services team cares for terminally ill patients who choose comfort-oriented care over curative treatment. Hospice care emphasizes quality of life and is provided wherever the patient calls home—whether a private residence, a relative’s home, or a nursing facility. Our interdisciplinary team of professionals and volunteers works together to relieve physical symptoms and address psychological, social, emotional, or spiritual distress for patients and their caregivers. 24-hour on-call nursing services are available for urgent needs.

This part-time, non-exempt position provides direct patient care every other weekend (Friday 5pm to Monday 8am). The role involves assessing patient health needs, developing and implementing care plans, administering nursing care, and maintaining medical records. The Weekend Nurse (RN) advises patients on health maintenance, disease prevention, and provides case management while ensuring confidentiality.

Responsibilities

• Work independently and collaboratively to provide end-of-life care and instruction for patients and families based on established clinical nursing practice standards.
• Direct end-of-life care, symptom management, and medication administration in compliance with physician’s orders and the plan of care; inform the primary medical provider of any changes in patient condition or needs.
• Monitor and interpret symptoms and treatments; develop a plan of care in collaboration with an interdisciplinary team.
• Provide education and support to patients and families.
• Assess and determine the physical, social, and environmental needs of clients; develop a treatment plan with other team members.
• Initiate the plan of care and set priorities to ensure effective and efficient patient care delivery and achievement of desired outcomes.
• Consult and coordinate with interdisciplinary team members to assess, plan, implement, and evaluate the patient’s plan of care.
• Follow a plan of care that identifies, prioritizes, and meets patient and family needs to achieve clinical goals.
• Regularly re-evaluate the patient’s nursing needs and revise the plan of care as necessary.
• Ensure care, educational, and end-of-life planning needs of patients are met.
• Direct and supervise Licensed Practical Nurses (LPN) and Certified Home Health Aides (CHHA), including supervisory visits.
• Communicate patient care reports to appropriate team members, including any changes in patient condition to physicians and families.
• Demonstrate knowledge and skills to provide age-appropriate care, assessing data reflective of the patient’s status and interpreting needs across the lifespan.
• Document care in electronic medical records per standards, including observations, activities, and clinical condition in a timely manner.
• Provide hygiene and personal care to patients.
• Maintain knowledge of and compliance with Medicare/Medicaid, state regulations, HIPAA, and accreditation requirements for hospice services.
• Coordinate regularly with team members to maximize effectiveness and efficiency of operations.
• Participate in program quality improvement projects.
• Keep supervisors informed of work progress, challenges, and suggestions for improvement.
• Rotate on-call duties to meet patient care needs after hours, including triaging phone calls, admissions, symptom management visits, education, and attending patient deaths.
• Perform other duties as assigned to support RiverStone Health’s mission and goals.

Requirements

• Current license as a Registered Nurse in Montana.
• Valid Montana Driver’s License and vehicle insurance.
• Current BLS certification.
• Associate degree in Nursing (minimum).
• Any combination of experience and training providing equivalent knowledge, skills, and abilities.
• Computer literacy in Microsoft Office Suite.
• Ability to maintain a calm and positive demeanor during difficult interactions.
• Non-judgmental and empathetic listening skills.
• High degree of detail-oriented skill.
• Knowledge of HIPAA and confidentiality requirements for protected health information.

• Bachelor of Science in Nursing.
• Experience with hospice or palliative care.

Pay

$34.52 to $50.04 hourly, depending on years of transferrable experience and internal equity.

Schedule

Part-time, non-exempt. Every other weekend: Friday from 5pm to Monday at 8am. Includes rotating on-call duties after hours, weekends, and holidays.

Physical Demands and Working Conditions
