Weekend Intake Registered Nurse, RN → Part-Time Registered Nurse

About the Role

This position involves evaluating, developing, and implementing policies to ensure high-quality patient care, with a focus on Care Navigation to guide patients through the healthcare system and address obstacles to receiving appropriate care. It requires working independently, managing processes, and applying advanced knowledge and experience.

Responsibilities

• Assess patient eligibility for home health services, including safety and insurance requirements.
• Coordinate referrals, obtain necessary documentation, and prioritize cases based on clinical and insurance factors.
• Communicate with referral sources, insurance providers, and community resources to facilitate patient care.
• Manage pre-authorizations, pre-certifications, and documentation to support the initiation of services.
• Utilize critical thinking to resolve referral issues and ensure safe, appropriate patient placement at home.
• Maintain compliance with regulations, keep management informed of workflow and referral patterns, and identify opportunities for new referrals.

Requirements

• Active RN license in the state of service with at least 2 years of visiting nurse experience.
• Strong organizational, communication, and problem-solving skills.
• Ability to function effectively in busy environments and handle multiple priorities.
