Responsibilities

  • Provides assigned nursing and personal care tasks to help clients and families maintain a healthy, safe home environment.
  • Assists with hygiene, nutrition, homemaking, and childcare tasks as part of the client's overall care.
  • Follows the nursing, OT, or PT-initiated plan of care; contacts supervisor for approval before deviating from the plan.
  • Maintains accurate, up-to-date electronic documentation of care provided.
  • Follows agency protocols including home-visiting standards, bag technique, infection control (MDROs, universal precautions), and attendance/appearance standards.
  • Communicates and collaborates effectively with the interdisciplinary team.
  • Participates in weekend and holiday rotation and other assignments based on agency needs.
  • Demonstrates knowledge of Hospice philosophy of care and complies with Federal, State, and Agency policies.
  • Represents the agency professionally, treating clients, families, and staff with respect.

Requirements

  • High school diploma or GED equivalent.
  • Valid Vermont LNA License.
  • Minimum of one year of experience as an LNA.
  • Valid state driver's license and proof of auto insurance.
