Responsibilities
- Provides assigned nursing and personal care tasks to help clients and families maintain a healthy, safe home environment.
- Assists with hygiene, nutrition, homemaking, and childcare tasks as part of the client's overall care.
- Follows the nursing, OT, or PT-initiated plan of care; contacts supervisor for approval before deviating from the plan.
- Maintains accurate, up-to-date electronic documentation of care provided.
- Follows agency protocols including home-visiting standards, bag technique, infection control (MDROs, universal precautions), and attendance/appearance standards.
- Communicates and collaborates effectively with the interdisciplinary team.
- Participates in weekend and holiday rotation and other assignments based on agency needs.
- Demonstrates knowledge of Hospice philosophy of care and complies with Federal, State, and Agency policies.
- Represents the agency professionally, treating clients, families, and staff with respect.

Requirements
- High school diploma or GED equivalent.
- Valid Vermont LNA License.
- Minimum of one year of experience as an LNA.
- Valid state driver's license and proof of auto insurance.
- Ability to pass pre-employment driving record, criminal, adult/child abuse background, and OIG checks.
