Make an Impact! Be the voice and guide! You are the key to orchestrating your patients' care across the healthcare continuum. You'll work with physicians, peers, community colleagues, and others to identify opportunities to continually improve patient care experiences and services, improve population health, reduce cost, and achieve collaborative practices that exemplify Main Line Health System's commitment to patient-centered care, community engagement, and population health. Use your excellent communication and advocacy skills to partner with patients, families, physicians and colleagues to develop, implement, and monitor a comprehensive plan of care which includes health promotion, disease prevention, and disease management activities.
Responsibilities

- Partner with patients, families, physicians and colleagues to develop, implement, and monitor a comprehensive plan of care which includes health promotion, disease prevention, and disease management activities.
Requirements
- Three years of recent clinical experience required
- Bachelor's degree in Clinical Discipline required
- Active PA RN license required
