Weekend Baylor RN Registered Nurse - Home Health (Part-Time)
Empath Home Health, a member of Empath Health, is seeking a compassionate part-time Weekend Baylor RN Registered Nurse (Home Health) to deliver individualized care to patients in their homes throughout North Hillsborough & East Pasco counties.

Locations: Home health visits in North Hillsborough & East Pasco counties (including North Tampa, Lutz, New Tampa, Tampa Palms, Wesley Chapel, Land O’ Lakes, Dade City, San Antonio, and surrounding areas).
Schedule
This is a part-time Weekend Baylor role requiring full weekends: Saturday and Sunday (20 hours per week).
Responsibilities
- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Deliver skilled nursing services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless, holistic care
Requirements
- Graduate of an approved nursing program with an active Florida RN license
- Prefers BSN or MSN degree
- Minimum of 1 year of direct patient care experience
- Home Health RN experience required
- Strong assessment, critical thinking, and patient education skills
- Willingness to participate in weekend and holiday on-call rotations
