About the role

The Weekend Admissions Screener will screen referrals for admission to Liberty Commons Skilled Nursing Facility, complete new patient sign‑ins, and work closely with patients and families to determine eligibility and improve the admissions process.

Responsibilities

• Screen referrals using electronic platforms EPIC and 4NEXT for admission to Liberty Commons Skilled Nursing Facility.
• Assist in the admissions decision‑making process by evaluating patient applications.
• Provide patients and families with information regarding admissions requirements and processes.
• Complete new patient sign‑ins.

Qualifications

• Dedicated professional with experience in a similar role preferred but not required.
• Exceptional communicator and strong advocate for patient and family needs.
• Team player who collaborates effectively with interdisciplinary teams.
• Clinical background preferred but not required.

Pay

$37–$50 per hour (based on an 8‑hour day).

Schedule

Work every weekend (Saturday & Sunday); onsite preferred with remote/hybrid option available. Typical shifts are 8 am–4 pm or 9 am–5 pm.
