Weekend Admission Nurse (RN) (Full Time & Part Time Available)
About the role
Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including: Conduct Start of Care (SOC) Admissions - OASIS Conduct follow ups, evaluation visits as needed in designated territory Make home visits to patients in designated territory Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures Monitor clients’ conditions, reporting changes to Clinical team Collaborate with physicians and agency staff to deliver the best care possible Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided. Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.

Requirements
- Current and unrestricted CA RN license
- Current BLS/CPR Certification
- Valid CA Driver's License
- Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
- High level of clinical knowledge in home health care is highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired
- Previous experience with IV Medication infusions highly preferred
- Proficiencies initiating intravenous (IV) therapy is a plus

Qualifications
- Must be in the field visiting patients in their homes
- Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day)
- Also willing to consider Full Time: Saturday and Sunday plus 3 week days
