About the Role
Provides short-term residential care for homeless adults who are too ill or injured to recover on the streets or in shelters, but who do not need to stay in a hospital. This includes:
- A safe place to stay
- Meals
- Nursing care
- Help with medications
- Transportation to medical appointments
Responsibilities
- Operate an on-site clinic (in partnership with health providers) delivering primary care, mental health counseling, and substance-use support to address both medical and behavioral health needs together.
- Help residents secure stable housing by coordinating assessments, benefits enrollment, and housing placement, and continue support after residents leave the program to reduce returns to homelessness.
- Engage residents in an assortment of recreational activities on the weekends. Activity opportunities can include hosting a game night, showing a film, or free outings to cultural and sport events around Chicago. These activities may occur within the facility and/or function as a designated "field trip."
