Qualifications
- Bachelor’s degree preferred; may substitute relevant experience.
- Highly organized, detail-oriented with effective time-management skills.
- Proven ability to establish and maintain positive working relationships, consistently meet deadlines, demonstrate independent judgment and initiative.
- Experience using basic spreadsheets for data management.
- Willingness to provide support at a time, location, and in a manner that meets the needs of persons served.
- Working knowledge of federal benefits such as Social Security and Medical Assistance.
- Effective written and oral communication skills; Spanish/English fluency is a plus.
- Daily access to a reliable automobile and a valid driver’s license is mandatory.
- Must maintain a good driving record and automobile insurance throughout employment.
Responsibilities
- Assist clients with the DDA Waiver Enrollment Process and ensure they remain active in the program.
- Assist clients with Medical Assistance (MA) Re-Determinations.
- Maximize enrollment in DDA Waivers to secure federal reimbursement for the State.
- Contribute to the agency team working toward fulfillment of agency philosophy, mission, strategy, and annual goals and objectives.
- Assist with and participate in relevant training for Resource Connections staff, people served by the agency, and service providers.
- Complete mandated training per DDA regulations and demonstrate ongoing commitment to professional development.
- Coordinate the enrollment process for all persons served by the agency who are receiving services through one of the DDA waivers.
- Provide information and supports necessary for clients and their families to complete the enrollment process within given timeframes.
- Complete the eligibility screening process and provide specific information to clients and families regarding DDA’s Waivers.
- Coordinate the enrollment meeting, complete post-meeting paperwork, and assist with the completion of the Medical Assistance Application.
- Gather documentation to support the information listed on the client's Medical Assistance Application.
- Assist enrolled clients with maintaining their enrollment, including annual Medical Assistance Re-Determination.
- Work collaboratively with the Eligibility Determination Division (EDD), the State Review Team (SRT), and DDA to resolve eligibility issues.
- Assist with Added Service enrollments and update DDA regarding the status of each client enrolled in a DDA Waiver.
- Perform other duties as assigned that are consistent with the position and necessary for the efficient operation of the agency.
