How Will You Make An Impact & Requirements
The Vice President of House Calls is responsible for leading one of Mosaic's most critical value-based care programs, providing in-home clinical services to high-risk and medically complex patients. House Calls serves as a key wrap-around service that extends the primary care team into the home, improving patient outcomes, reducing avoidable utilization, and ensuring patients receive the right level of support at the right time.
- Drive exceptional patient care and operational performance through the management and growth of the House Calls program, with a particular focus on patients with advanced chronic illness, functional decline, and those requiring increased care coordination.
- Work closely with primary care providers, care management, palliative care, hospice partners, and interdisciplinary teams to deliver comprehensive, patient-centered care.
- Manage full operational and budgetary responsibility for the business unit, driving strategies to meet or exceed EBITDA targets.
- Develop and execute growth strategies that expand access to House Calls services for high-risk and medically complex patients.
- Maintain and improve operational performance through effective workforce planning, provider productivity, schedule optimization, and resource allocation.
- Develop and implement clinical initiatives and evidence-based pathways across Mosaic to enhance patient outcomes while reducing total cost of care.
- Lead the organization's strategy for delivering in-home care to high-risk, medically complex, and advanced illness populations.
- Partner with primary care providers, care management, palliative care, hospice partners, and regional leadership to ensure patients receive the appropriate wrap-around services as their needs evolve.
- Advance Mosaic's End-of-Life (EOL) strategy by establishing processes that identify and engage patients who would benefit from House Calls, palliative care, hospice, and other supportive care programs.
- Drive initiatives that improve quality of life, reduce avoidable emergency department visits and hospitalizations, and ensure care aligns with patient goals and preferences.
- Utilize risk stratification and population health data to prioritize outreach and intervention for the highest-risk patients.
- Communicate provider and business unit needs, plans, and priorities to the larger MPG organization.
- Work in conjunction with corporate staff to maintain productive, collaborative relationships with physicians and advanced providers.
- Partner with regional House Calls directors and regional medical directors to develop and implement effective clinical training programs that promote clinical excellence and consistent care delivery.
- Lead, mentor, and develop a high-performing, multi-state team of physicians, advanced practice providers, operational leaders, and support staff.
- Manage weekly leadership meetings to drive accountability, operational performance, and continuous improvement.
- Foster a culture focused on patient-centered care, clinical excellence, accountability, and operational execution.
- Communicate and support Mosaic’s mission, vision, and core values while executing enterprise strategy across all House Calls markets.
- Serve as a key operational leader within Mosaic's value-based care strategy, ensuring House Calls is fully integrated with primary care and other wrap-around services.
- Leverage expertise in value-based care models, including Medicare Advantage, MSSP, and High Needs ACO REACH, to maximize quality outcomes and financial performance.
- Partner with market leadership to optimize referral pathways and increase appropriate enrollment of high-risk patients into House Calls services.
- Monitor and improve performance against key quality, utilization, patient engagement, and financial metrics.
- Collaborate with regional and market leadership teams to educate providers on House Calls services, referral criteria, and patient selection.
- Develop strategies to increase provider engagement and strengthen House Calls as a trusted extension of the primary care team.
- Identify opportunities to expand House Calls services and support enterprise growth objectives.
- Ensure strict adherence to all federal, state, payer, and organizational regulations and requirements.
- Maintain a culture of compliance, quality, and patient safety across all House Calls operations.
Experience
A minimum of 5 years in a senior leadership role managing clinical teams, physicians, and APPs. Proven experience managing full P&L operations, including budgeting and EBITDA management.
Travel Requirements
Travel is required as needed to support clinical teams and operations across multiple markets including Florida and California.
