Vocational Nurse (PACT)

About the role

The Community Based Outpatient Clinic (CBOC) Licensed Vocational Nurse (LVN) supports the Patient Aligned Care Team (PACT), specialty, and mental health teams by providing Veteran-centered care. Under the supervision of an RN or provider, the LVN delivers clinical care, administers medications, collects patient data, coordinates care, and provides education while promoting safety, preventive health, and positive patient outcomes through compassionate, holistic care.

Responsibilities

• Implements nursing interventions to assist provider
• Consistently and correctly prioritizes patient care activities
• Identifies significant changes in the patient's condition and reports to the provider and/or RN Care Coordinator or charge nurse
• Implements nursing interventions based on scientific principles
• Performs complex therapeutic procedures requiring regular observation and guidance from the RN Care Coordinator or charge nurse
• Administers medications
• Collects specimens
• Performs minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy
• Administers IV piggybacks
• Recognizes acute patient's distress and initiates correct emergency interventions
• Maintains equipment and therapeutic environment to prevent injury to self or others
• Demonstrates compliance with infection control practices
• Performs processes associated with preparing reusable medical equipment
• Provides patient and family health education

Requirements

• Basic Requirements: Graduate of an approved school of practical or vocational nursing; full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth of the United States, or District of Columbia.
• Experience: Six months of qualifying experience as an LPN or LVN; or graduation from an approved school and one year of experience involving nursing care work in a supervised medical, nursing, or patient care facility; or completion of at least 1 year of qualifying experience at the GS-04 level or equivalent.
• Knowledge and Skill: Knowledge and ability to prepare, administer, and document actions taken specific to commonly prescribed medications; recognition of emergency measures and reporting of changes in patient condition; ability to perform complex therapeutic procedures with regular supervision.

Qualifications

• Education: Graduate of an approved school of practical or vocational nursing.
• Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth of the United States, or District of Columbia.
• Language Proficiency: Proficient in both spoken and written English.
• Experience: At least 1 year of qualifying experience at the GS-05 level or equivalent.
