About the role
Patient Care: Visits patients in their homes to assess their needs, evaluate symptoms, and implement a treatment plan. Collaborates with the palliative care interdisciplinary team and other agencies.
- Initiates or facilitates referrals to appropriate resources.
- Completes an average of two nursing visits per day.
- Assesses and manages pain and comfort.
- Triage symptom management via telephone.
- Communicates patient needs to relevant disciplines.
- Collaborates with physicians and pharmacists.
- Documents all care provided in the EMR.
- Ensures ECF chart compliance.
- Enters and documents all orders and notes.
Responsibilities
Teaches patients and families about self-care, medication indications, side effects, and contraindications. Assists in providing continuity of care.
- Teaches patient self-care.
- Teaches family about patient condition and care.
- Assists in understanding medical illnesses and plan of care.
- Serves as a preceptor to other staff.
Requirements
- Current nursing licensure in Indiana.
- CPR certification.
- Ability to perform physical tasks including standing, walking, sitting, climbing stairs, stooping, kneeling, twisting, bending, reaching overhead, and using both hands, wrists, and fingers.
- Ability to communicate verbally and in writing.
- Ability to observe patients and understand bodily functions.
- Ability to work in various environmental conditions.
- Valid driver's license and current auto insurance.
Qualifications
- Prior experience in palliative care/hospice nursing.
