Virtual Neurology Care Nurse Practitioner (Remote) NJ
We are seeking a skilled and compassionate Nurse Practitioner licensed in New Jersey to join our virtual care team. This is a remote, patient-facing clinical role delivering care to Local Infusion patients while partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment. You must live in and be licensed in New Jersey and will work remotely from NJ.
About the Role
Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth. Partner with neurologists and the broader care team to ensure continuity of care.
Responsibilities
- Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians
- Educate patients and families on diagnoses, treatment plans, and disease progression
- Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
Requirements
- Active, unrestricted Nurse Practitioner license in New Jersey
- 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
- Strong clinical judgement and clinical documentation skills
- Comfortable managing a full patient panel independently with physician collaboration
- Excellent communication skills with patients facing cognitive decline and their caregivers
Qualifications
- Certification in geriatrics, adult-gerontology, or family practice preferred
