Virtual Neurology Care Nurse Practitioner (Remote) NH

Local Infusion is seeking a Nurse Practitioner licensed in New Hampshire to join our virtual care team. This is a remote, patient-facing clinical role delivering care to patients with neurological conditions, memory disorders, and dementia via telehealth. You will work collaboratively with supervising physicians and the broader care team to increase patient access and reduce time to treatment. You must live in and be licensed in New Hampshire and will work remotely from NH. Responsibilities Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealthConduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physiciansPartner with neurologists and the broader care team to ensure continuity of careEducate patients and families on diagnoses, treatment plans, and disease progressionMaintain accurate, timely clinical documentation in the EMR consistent with regulatory requirementsManage a full patient panel independently with physician collaboration Required Qualifications Active, unrestricted NP license in New Hampshire2+ years of clinical experience in neurology, memory care, and/or dementia care settingsStrong clinical judgment and clinical documentation skillsExcellent communication skills with patients facing cognitive decline and their caregivers Preferred Qualifications Certification in geriatrics, adult-gerontology, or family practice Benefits Potential for performance bonusesMedical, dental, and vision insurance through employer planShort and long-term disability coverage401(k) with employer match15 days PTOCompetitive paid parental leave and flexible return to work policyProfessional development opportunities
