Key Accountabilities
The Local Infusion Way
Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth.
Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies.
Partner with our neurologist(s) and broader care team to ensure continuity of care.
Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements.
- Active, unrestricted NP license in Connecticut; certification in geriatrics, adult-gerontology, or family practice preferred
- 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
- Strong clinical judgement and clinical documentation skills
- Comfortable managing a full patient panel independently with physician collaboration
- Excellent communication skills with patients facing cognitive decline and their caregivers
