Virtual Family Nurse Practitioner - Licensed

Role Overview

The Virtual Medical Team (VMT) provides evidence-based, 24/7 virtual primary care, treating patients via tele-health including video visits, phone calls, and email follow-ups. The role involves continuous learning through clinical rounds, collaboration with virtual and in-office team members, and delivering exceptional patient care using clinical training. Flexibility to obtain additional state licensure is required.

Responsibilities

• Conduct tele-health visits and triage calls to assess and manage patient health
• Collaborate with team members through daily huddles and shared communication platforms
• Utilize clinical expertise to deliver high-quality virtual care
• Participate in ongoing education and clinical rounds
• Obtain necessary licensure for virtual primary care in additional states

Qualifications & Skills

• Completed an accredited FNP program with national certification
• At least 2 years of outpatient primary care or urgent/emergency care experience with patients of all ages
• Licensed in CA with ability to secure additional state licenses
• Strong clinical, communication, and technology skills
• Ability to work flexible shifts including evenings, weekends, and every other Saturday or Sunday
• Passion for human-centered primary care and effective patient communication
