Virtual Clinician Home Evaluations Nurse Practitioner or Physician Assistant Full Time - Michigan
About the role
We're looking for full-time, board-certified nurse practitioners or physician assistants to join our team at Signify Health. You'll help connect millions of people across the country with the right care through online Video Visits.
Responsibilities
- Conduct approximately 30-minute Video Visits with health plan members, focusing on medication and medical history reviews, and answering questions.
- Work in a private space with adequate internet connection.
- Provide 40 hours of availability weekly, aiming for 320+ capacity units per month.
- Ensure a strong visit completion rate, completing 220+ visits per month.
- Maintain training, affiliation, and compliance with Video Visit requirements, including regular training.
Requirements
- Trained and credentialed for all health plan clients and lines of business in primary coverage state(s).
- Accountable to Signify Health's operational, clinical, and member experience key performance indicators and metrics.
- Maintain active state licenses in coverage areas (FNP, AGNP certification preferred for NPs).
- Board certified.
- Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, and telehealth visit delivery (preferred).
- Able to obtain and maintain multi-state licenses in coverage area, as required.

- Communicate effectively via phone, email, and electronic platforms.
- Practice autonomously while maintaining professional standards and ethical conduct.

- Effective verbal, written, and electronic communication skills.

Pay and Benefits
The typical pay range for this role is $87,035.00 - $187,460.00. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors. Our people fuel our future.
