Responsibilities
- Perform initial and ongoing client assessments for personal care services
- Develop, review, and revise individualized service plans based on client needs
- Conduct supervisory visits at least every 90 days and as needed when client condition or care needs change
- Supervise and support the delivery of personal care services by caregiving staff, including CNAs, HHAs, PCAs, homemaker aides, companions, and sitters
- Support caregiver onboarding, competency verification, and in-service training compliance
- Document client assessments, service plans, supervisory visits, training, and care coordination notes in a timely manner
- Communicate changes in client condition, care needs, or safety concerns to appropriate agency personnel
- Carefully coordinate services with the Executive Director and Director office team, client/family, and other care partners as appropriate
- Participate in quality assurance, performance improvement, record review, and compliance activities as assigned
- Make abuse, neglect, or exploitation reports in accordance with state requirements and agency policy
- Notify appropriate parties of changes in client condition or needs, including physicians or referral sources when applicable
- Remain accessible as needed for questions or concerns related to personal care services during assigned/covered periods
- Follow all CareBuilders at Home policies, confidentiality standards, and Virginia non-medical home care regulations
Requirements
- Active, unrestricted Registered Nurse license in Virginia required
- Graduate of an accredited school of professional nursing
- Minimum 2 years of nursing experience preferred
- Prior home care, home health, senior care, or supervisory experience preferred
- Strong documentation, communication, and care coordination skills
- Must be able to complete background check, TB screening, CPR certification, and required onboarding documentation
- Must be able to read, write, understand, and speak English
- Must be physically able to perform the essential functions of the role
Qualifications
- Ability to travel to client homes as needed
