As an APRN, you will provide exceptional medical care to patients 65+ by conducting physical exams, ordering and interpreting diagnostic tests, and developing patient treatment plans under physician supervision. This is a safety-sensitive position involving direct patient care and medication administration.

Responsibilities

• Practice medicine under physician supervision, adhering to ethical standards and staying current with industry technologies and best practices.
• Provide care for older adult patients with acute and chronic illnesses, ensuring appropriate consultation with specialists while maintaining responsibility for detailed patient care.
• Collaborate with hospitalists when patients are hospitalized.
• Ensure compliance with all clinical obligations, including accrediting agency requirements, CLIA, drug prescription regulations, HIPAA, and state/federal safety regulations.
• Assess, diagnose, treat, and manage patients with acute and complex chronic health needs using evidence-based medicine and practice guidelines.
• Follow all internal policies and procedures to ensure appropriate clinical processing and documentation.
• Maintain complete and detailed electronic medical records (EMR) of patients’ health care plans and prognosis within 48 hours of each visit.
• Educate patients on health promotion, risk reduction, and disease/disability management.
• Analyze and interpret patient histories, symptoms, physical findings, and diagnostic data (e.g., blood gases, radiology, EKG) to develop appropriate diagnoses and treatment plans in collaboration with the supervising physician.
• Exhibit compassionate, high-quality, and conscientious behavior while building effective relationships with patients, families, and team members.
• Document continuing education per licensure and certification requirements.
• Ensure patients are seen by the physician every other visit.
• Maintain patient confidentiality in accordance with policy and HIPAA requirements.
• Follow the proper chain of command for patient complaints.
• Perform additional duties as assigned.

Requirements

• Graduate of an accredited school of nursing.
• Certified as a nurse practitioner with current state licensure.
• At least 1 year of professional nursing experience, with 1–3 years of experience as a primary care nurse practitioner.
• Prior experience working with an electronic medical records (EMR) system.
• ACLS/BLS certification as mandated by state regulations.
• Demonstrated knowledge of appropriate patient care rationale and ability to handle emergency situations calmly and effectively.
• Strong written and verbal communication skills, including teaching ability and comfort with presenting information to diverse groups.
• Proficiency with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration.

Physical Requirements

• Full range of body motion, including handling and transferring patients, manual and finger dexterity, and eye-hand coordination.
• Ability to stand, walk, push, bend, kneel, and reach at arm’s length and overhead for prolonged periods in a clinic setting.
• Capability to lift and move patients and medical equipment, including lifting/carrying up to 15 lbs.
• Corrected vision and hearing within normal range.
• Ability to sit for extended periods.
• Ability to operate a motor vehicle with personal transportation.
