LPN - Village Health Services Cross Keys Village - The Brethren Home Community (CKV) is currently seeking a Licensed Practical Nurse to become a part of our team and help provide our villagers with the highest quality of care. Responsibilities
- Successfully developing and implementing a health care program to evaluate Villager’s level of independence and need for supportive services.
- Determining the needs of especially frail or ill Villagers and helping to establish linkage with supportive services.
- Leading the discharge planning process when Villagers can no longer live safely in a cottage/apartment.
- Facilitating communication between members of an interdisciplinary team when considering Villagers’ care plans.
- Evaluating villagers’ needs and providing care as needed to ensure the health, welfare and safety of the Villagers.
- Responding to emergencies and evaluating needs, providing nursing care as needed.
- Collaborating with physician and/or RN as needed.
- Assuring that Villager’s rights are protected by the Resident’s Bill of Rights.
- Completing required documentation and record keeping as required.
- Reviewing pre-admission and completing new admission evaluations per Village Health Services standards for admission.
- Collaborating with Villagers, team members and outside agencies to identify, establish and arrange for appropriate services.
- Conducting new Villager orientation on health and medical services as well as safety and well-being.
- Coordinating the provision of available health care services to help assure the health, welfare and safety of the Villagers.
- Referring Villagers to physicians or appropriate agency as medically appropriate.
- Planning and evaluating the Villager’s need for nursing or Village Health Navigator regarding his/her socialization.
Requirements
- Must be licensed as a practical nurse in the State of Pennsylvania.
