Vascular Case Manager
Patient Case Ownership
Assume ownership of all approved intervention candidates following Eligibility Nurse review.
Track patient progress from eligibility approval through procedure completion.
Identify barriers preventing patients from advancing through the procedural pipeline.
Escalate concerns requiring physician, clinical, or operational intervention.
Ensure all patients remain actively progressing toward procedural readiness.
Surgical Readiness Coordination
- Verify completion of required laboratory testing.
Coordinate repeat or missing laboratory draws as necessary.
Monitor and track diagnostic imaging requirements.
Ensure appropriate medication hold orders have been obtained and communicated.
Verify physician clearances, specialist clearances, and supporting documentation when required.
Coordinate acquisition of medical records, hospitalization records, and outside provider documentation.
Facilitate communication regarding anticoagulation management and other procedural preparation requirements.
Facility & Family Coordination
- Collaborate with facility staff to ensure procedural preparation requirements are understood and completed.
Coordinate communication with facility nursing leadership regarding patient readiness.
Work with responsible parties, family members, guardians, and powers of attorney regarding procedural preparation and consent requirements.
Address transportation concerns and logistical barriers affecting procedural scheduling.
Assist in resolving patient or facility concerns that may delay treatment.
Interdepartmental Collaboration
- Serve as the primary communication bridge between:

Participate in regular readiness meetings and procedural planning discussions.
Collaborate with Eligibility Nurses to identify high-risk or complex patients requiring enhanced oversight.
Partner with Surgical Schedulers to prioritize patients based on readiness status and physician availability.
Pipeline Management & Reporting
- Maintain accurate patient status documentation within company systems.
Monitor patient movement through established procedural workflow stages.
Generate and maintain readiness reports and patient tracking tools.
Identify trends causing delays, cancellations, or procedural inefficiencies.
Provide regular updates to leadership regarding patient pipeline status.
Assist leadership in forecasting procedural volume and readiness metrics.
Quality & Compliance
- Maintain patient confidentiality and HIPAA compliance at all times.
Ensure procedural preparation requirements are completed in accordance with physician protocols and company policies.
Assist with quality improvement initiatives related to patient throughput and procedural readiness.
Maintain accurate documentation and communication records.
Support compliance with applicable state, federal, and accrediting body requirements.
Qualifications
- Minimum 3 years of healthcare coordination, case management, surgical scheduling, vascular operations, or related clinical administrative experience.
Strong understanding of medical terminology.
Experience coordinating care across multiple departments and stakeholders.
Exceptional organizational and time management skills.
Ability to manage multiple patient cases simultaneously.
Strong written and verbal communication skills.
Proficiency in Microsoft Office Suite and healthcare information systems.
Demonstrated problem-solving and critical-thinking abilities.
Preferred
- Licensed Practical Nurse (LPN).
- Registered Nurse (RN).
- Certified Medical Assistant (CMA).
- Surgical Scheduling experience.
- Vascular surgery, interventional radiology, cardiology, wound care, or procedural medicine experience.
- Experience working with geriatric patient populations.
- Experience working with skilled nursing facilities and assisted living facilities.
