VA Self-Directed Care Counselor
About the role
The VA Self-Directed Care Person-Centered Counselor at the Area Office on Aging of Northwestern Ohio (AOoA) helps veterans, older adults, individuals with disabilities, and caregivers maintain independence and support in their homes and communities. This role involves conducting assessments, coordinating services, and ensuring compliance with program requirements.
Key Responsibilities
- Conduct initial and annual assessments to evaluate needs and service gaps
- Aid with enrollment documentation and program onboarding
- Develop and monitor care plans and spending plans
- Coordinate with VA and Financial Management Services (FMS)
- Provide education on program requirements and employer responsibilities
- Maintain participant health, safety, and service delivery
- Accurately document within 24 hours
- Submit required monthly reports
- Support participant rights, choice, and self-determination
- Travel throughout the service area as needed
Qualifications
- Licensed Social Worker (LSW) or Registered Nurse (RN) in Ohio (active and unencumbered)
- At least one (1) year of experience in home health, case management, social work, behavioral health, or related field
- Prior experience conducting health and social assessments
- Knowledge of community resources, family dynamics, and human behavior
- Strong communication, organization, and problem-solving skills
- Proficiency in Microsoft Word and Excel
- Ability to work independently and as part of a team
Schedule & Work Environment
This role is a four-day work week (Monday–Thursday or Tuesday–Friday) based in Toledo, Ohio, with travel required. Additional expectations include maintaining HIPAA compliance and confidentiality, participating in required trainings, demonstrating cultural competency, adhering to agency policies and procedures, and lifting/carrying up to 20 pounds.
Physical Requirements
A valid driver’s license, reliable transportation, and insurance are required.
Special Requirements
- Background check, drug/alcohol screening, and Motor Vehicle Record (MVR) check are required
- Cross-training may be required
- Position is contingent upon funding
