Urgently Hiring! Speech-Language Pathologist Fee for Service

About the Role

Gurwin Healthcare System is seeking a per diem Speech Language Pathologist for our Home Care Division. This role involves providing speech/language and dysphagia rehabilitation services to adult patients in their homes, primarily covering Nassau County and/or Eastern Suffolk, though other towns may be considered.

Responsibilities

• Plan and provide speech/language and dysphagia rehabilitation services to patients in their homes.
• Work with patients and families to provide training and/or assistive devices for rehabilitation.
• Report patient’s initial status, significant changes, therapy plans, goals, ongoing status, and discharge plans.
• Provide timely documentation, including assessments, plans, goals, visit care, summary reports, and billing/visit data.
• Instruct patients, families, and team members on programs that enhance care and follow-up.
• Evaluate the patient’s social and architectural environment, making and implementing recommendations as feasible.
• Comply with all regulatory criteria and notify the agency of visit schedules and changes.
• Provide professional leadership, clinical guidance, and expertise to team members.
• Attend regular continuing education programs.
• Adhere to all aspects of customer service, including teamwork, empathy, courteous communication, and respect for others.

Requirements

• Current New York State License to practice Speech Pathology.
• Minimum of two years of experience as a Speech Pathologist.
• Home health experience is preferred.
• Ability to travel to patients' homes to make home visits.
• Ability to make clinical and managerial decisions within the scope of professional practice to perform speech therapy procedures competently and independently.

Pay

• $80 for initial visit
• $65 for follow-up visit
• $70 for discharge visit
