About the Role
Human Touch Home Health is seeking a compassionate and skilled Occupational Therapist (OT) to join our growing team serving patients throughout Southern Prince George's County, Maryland, including Clinton, Fort Washington, Oxon Hill, Temple Hills, Suitland, Camp Springs, District Heights, Forestville, Marlow Heights, Accokeek, Friendly, and surrounding communities. As a Home Health Occupational Therapist, you'll help patients regain the skills, strength, and confidence they need to safely perform everyday activities while remaining in the comfort of their own homes. You'll develop individualized treatment plans, build meaningful relationships, and celebrate milestones alongside your patients in a personalized, one-on-one care setting.

Responsibilities
- Complete occupational therapy evaluations, reassessments, and discharge visits
- Develop individualized treatment plans based on patient goals and functional needs
- Provide skilled occupational therapy services in patients' homes
- Assess home environments and recommend adaptive equipment and safety modifications
- Educate patients and caregivers on home safety, adaptive equipment, energy conservation techniques, and activities of daily living
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians, nurses, and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
Requirements
- Active Maryland Occupational Therapist License
- Valid Driver's License and reliable transportation
- Strong clinical assessment, communication, and documentation skills
- Home health experience preferred but not required
- OASIS experience preferred
- Experience with WellSky Home Health is a plus
