About the role

The Resident Care Coordinator (RCM) oversees patient care, implements and coordinates services, and manages nursing and certified nursing assistant staff.

Responsibilities

  • Supervise and implement patient care through interpersonal contact with patients, families, nursing staff, and others.
  • Manage and coordinate patient care and services.
  • Prepare and administer medication as ordered by a physician.
  • Coordinate patient care plans and services.
  • Evacuate patients as needed.
  • Communicate effectively and work productively on an interdisciplinary team.

Requirements

  • Nursing diploma (associate’s or bachelor’s degree in nursing).
  • Currently licensed/registered in applicable State with an active RN license in good standing.
  • One (1) year geriatric nursing experience preferred.
  • CPR certification upon hire or obtain during orientation.

Qualifications

  • Advanced knowledge in field of practice.
  • Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility.
  • Proficient in implementing and interpreting programs, goals, objectives, policies, and procedures of the department.
  • Maintain professional working relationships with all associates, vendors, etc.
  • Maintain confidentiality of all proprietary and/or confidential information.

Pay

  • $48.00 - $58.07 per hour DOE.
  • Sign-on bonus of $7,500.
