About the role
The LPN Unit Care Coordinator supervises and manages patient care, ensuring adherence to laws, regulations, and standards.
Responsibilities
- Supervises and manages patient care
- Ensures adherence to laws, regulations, and standards
- Serves as a liaison between patients, families, and healthcare providers
- Administers medications accurately
- Responds to emergencies
- Coordinates services with RNs
- Maintains confidentiality and adheres to ethical standards
- Implements departmental policies
- Effectively leads daily nursing activities
- Manages patient care plans
Requirements
- Completed an accredited LPN program
- Holds an active, in-good-standing LPN/LVN license
- Preferably has at least one year of geriatric nursing experience
- CPR certification required upon hire or during orientation, and must remain current
- Possesses advanced knowledge of practice, makes independent decisions, and understands relevant laws and procedures
- Demonstrates professionalism, excellent communication, teamwork, and dependable attendance
- Fluent in English, with strong reasoning, judgment, and interpersonal skills
