Triage Nurse
About the role
Delaware Valley Community Health has been serving the underserved population of Greater Philadelphia for over 55 years. We are looking for a Triage Nurse to join our Clinical Team at our busy multi-cultural ambulatory care centers.
Responsibilities
- Assess patients and establish the level of care needed both virtually and in-person.
- Collect and interpret patient data in consultation with a provider.
- Record all patient data in the patient electronic health record (EHR) after each encounter.
- Initiate diagnostic and therapeutic measures as indicated by standing protocols.
- Provide health counseling and instructions regarding common patient problems.
- Work with a Clinical Pharmacist in supporting patient education regarding medications and adherence to appointments.
Requirements
- Graduation from an accredited school of nursing.
- Current PA license as a Registered Nurse (RN).
- CPR Certification.
- Prior experience in medical/surgical, critical care, emergency care, or ambulatory care.
- Strong critical thinking, collaboration, and communication skills.
- Ability to handle multiple in-bound calls and phone lines.
- Proficiency in using technology for patient care and connected clinical devices.
- A positive and enthusiastic demeanor when interacting with patients and staff.
- Bilingual Spanish/English preferred.

Pay
Commensurate with experience.
Schedule
Monday - Friday / No weekend clinic hours.
