TREATMENT TEAM COORDINATOR / PROGRAM ADMIN SPECIALIST I
Job Summary
The Treatment Team Coordinator / Program Admin Specialist I is responsible for coordinating the development of comprehensive and individualized treatment plans that address mental health, substance use, and medical/nursing care needs. This role involves monitoring and ensuring the accurate and timely completion of treatment care plans and related documentation.
Responsibilities
- Develop and monitor individualized treatment plans that align with current professional practice standards.
- Interpret and utilize clinical documentation standards, such as DSM-V, Physician’s Drug Reference Book, TJC, and CMS standards, to ensure consistency in care.
- Utilize evidence-based psychosocial treatments, including stages of change, motivational interviewing, and shared decision-making tools, to promote person-centered treatment plans.
- Compose and evaluate non-pharmacological interventions tailored to meet the biopsychosocial needs of patients with varying levels of acuity and complexity.
- Use Microsoft Office programs (Word, Excel, Outlook) and Electronic Health Records to manage patient data and documentation.
Qualifications
- Experience and knowledge of psychiatric treatment for adults and geriatrics.
- Experience working on interdisciplinary teams and developing treatment care plans.
- Proficiency in clinical documentation standards and interpretation of relevant resources.
- Experience with evidence-based psychosocial treatments and motivational interviewing techniques.
- Extensive experience using Microsoft Office programs and Electronic Health Records.
- Preferred experience in an inpatient psychiatric hospital setting.
