Traveling Social Service Specialist

Responsibilities

• Aids clients in adjusting to nursing home placement
• Documents progress and updates plans of care at interdisciplinary care conferences
• Arranges transportation, makes appointments, and acts as a liaison between all departments, residents’ families, and outside agencies
• Assists in handling and accepting placement through one-on-one counseling and group work
• Ascertains potential and develops discharge plans when clients are admitted
• Arranges for post-discharge services and follow-up care
• Assesses emotional, financial, spiritual, educational, and social needs of residents
• Ensures those needs are met
• Acts as resident/family advocate and ensures the resident is knowledgeable in and exercises his/her rights
• Organizes and conducts Family Counseling Meetings and maintains minutes of the meeting
• Maintains effective communication with residents, families, health care professionals, and facility staff
• Assists with training and onboarding of new hires

Qualifications

• Bachelor’s degree in Social Work required
• Must hold and maintain an active Social Work license in good standing, in accordance with all applicable state licensing requirements
• Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals
• Demonstrated ability to build and maintain professional relationships and collaborate with external agencies to address health and welfare needs
• Ability to work effectively with diverse populations in a variety of settings, including community and long-term care environments
• Strong critical thinking skills with the ability to evaluate case information and make informed decisions regarding appropriate interventions and procedures
• Prior experience in long-term care required
• Must be able to travel up to 100% to assigned centers, including travel by car or air
