Job Manager
- Manages facility wound care program
- Provides direct patient care and assistance to staff nurses
- Acts as consultative service to affiliated healthcare agencies
- Develops and implements the facility wound care program
- Establishes standards of care, competencies, policies and procedures
- Consults on any wound that does not show measurable signs of healing within two weeks
- Reviews medical records of patients with wounds
- Ensures that wound assessments, care plans, and treatments are clearly and correctly documented
- Investigates all cases with adverse events related to wounds through the completion of root cause analysis (RCA)
- Participates in clinical outcome monitoring, follow-up and agency performance improvement initiatives
- Collaborates with Support Center Clinical Operations staff to maintain a cost-effective wound care formulary
Facility Requirements
- Must have 3 years recent Wound Care Coordination experience
- Must have ACLS, BLS certifications
- Must have WOCN, CWS or WCC certification
- Must have California RN license
