Certification Details

• BLS certification required for all positions having patient contact.
• ARDMS certification required.

Job Requirements

• License/Certificate verification required within 30 days prior to assignment start date.
• Proof of 2 MMR vaccinations or an IGG titer required prior to start.
• TB Test (QuantiFERON or T-Spot) completed within 12 months prior to assignment start date; for past positive, CXR and updated TB Questionnaire required annually.
• Proof of 2 Varicella vaccinations or an IGG titer required prior to start.
• Physical/Health Statement attestation required, signed by MD, DO, NP, or PA.
• Memorial Background Check within 30 days prior to assignment start.
• Employment verification for previous 7 years required.
• Two references from Manager/Supervisor level or above required.
• Memorial I-9 E-Verify attestation required.
• Drug screen (10 panel) completed within 60 days prior to start date.
• Education verification required for highest level of education.
• Memorial Badge Photo required (color photo, solid blue background, no selfies or filters).
• National Sex Offender check within 30 days prior to start, updated annually.
• OIG Sanction Search within 30 days prior to start, updated annually.
• OFAC Sanctions List Search within 30 days prior to start, updated annually.
• SAM gov check within 30 days prior to start, updated annually.
• Memorial Skills Checklist current within 12 months prior to assignment start date, required annually or at any break in service.
• Memorial Core Competencies completed within 365 days prior to start with 80% pass rate.
• Memorial Attestation Form acknowledging receipt of required policies.

Schedule Information

Sunday, Monday, Tuesday shifts from 7:00 PM to 7:30 AM.

Position floats between two Free Standing Emergency Departments and Memorial Hospital Pembroke Pines Emergency Department.
