• Provide stepdown care to patients requiring transitional care between acute and chronic care settings.
• Assist with patient education and discharge planning.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with interdisciplinary teams to ensure patient care continuity.

Requirements

• 2 years of experience in the unit required.
• BLS required.
