Job Requirements
- Must have professional license and/or certification as required for the position with primary source verification within 30 days of start date
- MA Conditional Licenses not accepted
- Documentation of required immunizations: MMR (measles, mumps, rubella) or proof of immunity
- Documentation of TB test within 3 months of start date and updated if >6 month break
- Physician health statement within 365 days of start date and updated if >6 month break
- Criminal background check within 45 days prior to start date and updated if >6 month break
- Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break
- Resume including past 7 years of work experience with city and state
- Two references within 4 years, minimum one supervisor or above in skillset
- Skills checklist completed within 1 year of submission
- Confirmation of assignment/POI form required for all initial assignments and extensions
- Education verification through background check company
- SSN trace with addresses/aliases for past 7 years
- National sex offender check within 30 days of start and updated annually
- Exclusion checks within 30 days of start and annually
- Name change documentation if applicable
- TB symptom survey if positive PPD, completed prior to start and updated annually
