Requirements

  • Master's degree from an accredited speech-language pathology program
  • Active and unrestricted NJ Speech-Language Pathology license (license required if applicable)
  • Minimum of 1 year of experience in an acute, subacute, or home care setting
  • Home health experience required
  • Strong customer service and communication skills
  • Use of a personal vehicle is required for home visits
  • 2 professional references required for consideration

Details

  • Home health setting with travel to patient homes and facilities
  • Monday through Friday, 8am to 4pm; no weekends
  • 40 hours per week, 8 hours per day
  • May be required to float to a nearby sister facility as needed
  • No on-call required

Additional Information

  • Perform baseline assessments of patient swallowing, communication, and cognitive abilities
  • Collaborate with patients, families, and physicians to develop and revise individualized plans of care
  • Administer speech therapy services per physician orders, including communication, safe swallow, and cognitive training
  • Absorb social environment and clinical risks, adjusting programs and providing education as needed
  • Provide discharge planning and referral to appropriate resources
  • Communicate and coordinate with physicians, nurses, and other clinical disciplines through IDT meetings
  • Document patient education, plan of care, and progress in accordance with agency policies and home care regulations
  • Demonstrate knowledge of home care regulations and insurance reimbursement guidelines
  • Manage caseload and prioritize scheduling to meet patient needs and productivity standards

Qualifications

  • Candidates must reside more than 50 miles from the facility to be eligible for this position

Pay & Benefits

  • Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation)
  • Tax-free stipend amount to be determined
