Overview

Social Worker – Behavioral Health in Tuba City, Arizona.

Responsibilities

  • Provide outpatient and inpatient social work services to identified high-risk patients;
  • Assess needs, develop care plans, and coordinate services;
  • Collaborate with a multidisciplinary team (physicians, nurses, case managers, care coordinators) to ensure a seamless continuum of care;
  • Conduct discharge planning, utilization review, and assist with admission decisions for ED presentations and other hospital events;
  • Link patients with community resources, behavioral health and substance use services, and follow through to achieve measurable outcomes;
  • Monitor patient progress, reassess care plans, and advocate for patient and family engagement to promote safety and self-management;
  • Document care processes and utilize the electronic health record to support case management and care coordination across settings.

Requirements

  • Master’s degree in Social Work (MSW);
  • Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with an active, unrestricted license in any U.S. state or territory;
  • Current Basic Life Support (BLS) certification from the American Heart Association;
  • Minimum three (3) years of clinical care experience; experience with discharge planning, case management, or utilization review;
  • Knowledge of electronic health record systems; ability to work effectively in inpatient and outpatient settings;
  • Ability to collaborate with a multidisciplinary team and work within a health system.

  • Weekly pay: $2,468–$2,493 per week;
  • Contract duration: 13 weeks;
  • Start date: 09/07/2026;
  • Shift: 5x8 Days (Mon–Fri, 8:00 a.m.–5:00 p.m.);
  • Setting: Surgical center with inpatient/outpatient social work;
  • Housing: Shared contractor housing options; housing application required 2 weeks prior to start; no pets or family allowed.
