Travel Social Work - Licensed Clinical Social Worker - $2,400 per week

LanceSoft is seeking a Social Work Licensed Clinical Social Worker for a travel job in Robbinsdale, Minnesota.
Responsibilities
- Provide clinical social work services to patients in a hospital setting.
- Conduct assessments and develop treatment plans based on patient needs.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Facilitate discharge planning and coordinate post-discharge services.
- Engage with diverse patient populations to provide culturally sensitive care.
Requirements
- Minimum of 2+ years of hospital Social Work experience.
- Experience with discharge planning.
- Experience working with diverse patient populations.
- Active Licensed Social Worker (LSW) license in the State of Minnesota.
- Epic Electronic Medical Record (EMR) experience.
Profile Requirements
- Complete work history.
- Current skills checklist relevant to the specialty, completed within the last 12 months.
- Two supervisory references with employment dates included.
- Candidate work authorization status must be clearly documented if not eligible to work in the USA.
- All requested time off (RTO) must be submitted at the time of candidate submission.
